Registration Fees and Policies
AAO-HNSF encourages all prospective attendees to register as soon as possible for the Annual Meeting to take advantage of reduced registration fees and to avoid waiting in long lines onsite. Attendees can register online until August 18, 2008.

Your registration fee for the Annual Meeting includes the following:

  1. Access to all Guest Lectures, Scientific (Clinical Research) and Translational & Basic Research Program Sessions
  2. Entrance to the OTO EXPO
  3. Access to the Complimentary Shuttle Service to and from the official AAO-HNSF hotels and the McCormick Place Convention Center
  4. Entrance to the Opening Ceremony and President’s Receptions

Your registration fee for the Annual Meeting does not include the following ticketed events:

  1. Instruction Courses
  2. International Dessert Reception
  3. AAO-HNSF/ANS Hands-on Workshops

Instruction Course Tickets
Early registration for Instruction Courses increases the possibility of receiving first-choice selections and saves you money. Instruction Course fees are $45 per hour, and $65 per hour for hands-on and computer courses, if you register in advance. Onsite Instruction Course fees increase to $65 per hour and to $85 per hour for hands-on and computer courses. Register online at www.entnet.org/annual_meeting.

Registration Types
Members: To be eligible for member rates, 2008 AAO-HNSF dues must be paid before registering. The AAO-HNSF will charge the higher, non-member registration fee if dues payments are not current and will not issue any refunds.

Member Candidates: These registration fees are available only to individuals who, prior to registering for the annual meeting, have had their completed membership application forms and fees accepted by the AAO-HNSF Membership Department.

Spouses: All registered spouses will receive tickets to the President’s Reception and are invited to attend the Scientific (Clinical Research) and Translational & Basic Research programs. Spouses are not eligible for Instruction Courses.

Non-Members: Non-Members must register to attend any portion of the AAO-HNSF Annual Meeting & OTO EXPO.

Media Registration: Members of the media who wish to register for the Annual Meeting can access AAO-HNSF’s Media Policies and Registration information at www.entnet.org/annual_meeting, beginning in August 2008. A fully equipped newsroom will be available to reporters onsite during the conference.

2008 Honored Countries - Registrants who reside in India, Russia, Ireland, and/or China will receive a $100 discount off registration fees. Regular instruction course fees will still apply.

Membership Address
Unless a permanent change in address is indicated on the registration form, the AAO-HNSF member addresses on file will be used in all communications.

Registration Confirmations
All registrants will receive emailed confirmation of their registration and Instruction Course enrollment. Registration materials will be mailed in advance to those who register by August 18, 2008, and reside in the U.S. or Canada.

There will be a 25 percent processing fee on cancellations received in writing on or before July 18. All other cancellation requests must be received in writing, on or by August 18 in order to be eligible for a 50 percent refund. All Instruction Courses are non-refundable. Letters of Invitation
Citizens of some foreign countries will need a visa to enter the United States. Registrants will receive information on how to obtain a visa letter with their annual meeting registration confirmation. Registration for the annual meeting is required before a letter of invitation can be issued. Contact invitations@entnet.org for additional information.

For security reasons, there have been several changes over the last few years for entering the United States. Advance travel planning and early visa application are important, since visa applications are subject to a greater degree of scrutiny than in the past. Visit the U.S. Department of State travel website to learn more about the application process and average wait times.

US-VISIT is part of a continuum of security measures that begins outside land borders, when a person applies for a visa to travel to the United States, and continues on through entry and exit at U.S. air and seaports and, eventually, at land border crossings.

The US-VISIT program enhances the security of U.S. citizens and visitors by verifying the identity of visitors with visas.

Policies

Electronic Devices and Recording Equipment
All cell phones, pagers, cameras, video cameras, and camera and video phones must be turned off during sessions. Recording of any kind is prohibited in all educational activities, events, and the OTO EXPO.

Smoking
Smoking is prohibited in all areas of the McCormick Place Convention Center.

Changes to Registration
No substitution of one individual for another is allowed. All changes or additions may be made online by August 18. After this date, changes must be made onsite.

Cancellation and Refund Policy
There will be a 25% processing fee on all cancellations received in writing on or before July 21. All other cancellation requests must be received in writing on or before August 18, in order to be eligible for a 50% refund.

Faxed and Mailed Forms and Receipts
The AAO-HNSF is not accepting faxed or mailed registration forms for the 2008 Annual Meeting & OTO EXPO. All registrations must be completed online at: www.entnet.org/annual_meeting. A confirmation will be emailed after registration is complete.

Security and Attendee Conduct Information
Admittance to all official 2008 AAO-HNSF events will require a proper display of registration badges. Badges should be worn and visible at all times and are nontransferable. False certification of individuals as paid AAO-HNSF Meeting Attendees, misuse of badges, any method of assisting unauthorized persons to gain access to any Annual Meeting Event, or inappropriate conduct will be just cause for repossessing badges of any individuals involved. As a result, AAO-HNSF reserves the right to expel all parties involved and withholds no obligation to refund.

Age restrictions
The OTO EXPO is a gathering of Otolaryngologists and industry professionals. Children 16 years of age and younger are not permitted on the Exhibit Floor.
